Consent
I am aged 18 and wish to take part in a Clip \‘n Climb session and /or I give consent for all under 18\’s in my booking to take part. I am the carer/guardian/ of these children or have the permission from their parents/Guardians to sign on their behalf.
I understand that photographs may be taken by staff during the session that may be used for media and marketing purposes.

Medical
Clip \‘n Climb can be a physically demanding activity.
I confirm that no-one taking part has any medical conditions that may be made worse by climbing at Clip \‘n Climb Clip n Climb Uxbridge. This includes any neck, back, shoulder, hip, and knee injury, anyone pregnant should take this into consideration as to whether they climb or not. If I have any concerns about myself or any member of the party I will seek advice from a member of staff. If in doubt DO NOT climb.

Risk
Clip \‘n Climb look after you by maintaining its equipment to a high standard, good quality staff training and providing insurance against a major incident or negligence.
Due to the physical nature of “Clip ‘n Climb” minor injuries such as bumps, bruises and scrapes are common and are to be expected as normal. I accept that minor injuries such as those outlined above and similar are likely and agree to indemnify Clip ‘n Climb Clip n Climb Uxbridge for claims relating to these under normal circumstances.
I understand that Clip \‘n Climb Clip n Climb Uxbridge is not responsible for the safe keeping or damage to belongings even when placed in the unsecure boxes provided for self-storage.

Instruction
I agree to follow the instruction that will be given to me in the Safety Briefing and to follow any verbal instructions from the Clip \‘n Climb staff during the session. I will make sure that I or any member of my party under the age of 18 years old fully understands these instructions before I/we take part.
